Ozzy Osbourne’s cause of death has been confirmed less a week after the heavy metal pioneer was laid to rest in his native England.
Osbourne’s death certificate, obtained by The New York Times on Tuesday, reveals the Black Sabbath frontman died from a heart attack. Coronary artery disease and Parkinson’s disease were listed as contributing factors.
Osbourne’s death at the age of 76 was announced by his family on July 22.
“It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” their statement read. “He was with his family and surrounded by love. We ask everyone to respect our family privacy at this time.”
The legendary rocker died just weeks after his farewell show with Black Sabbath in Birmingham, England, where he performed from a chair because he was physically unable to stand.
He first revealed in 2020 that he’d been diagnosed with Parkinson’s disease the previous year.
“It’s been terribly challenging for us all,” he said on Good Morning America at the time. “I did my last show New Year’s Eve at The Forum. Then I had a bad fall. I had to have surgery on my neck, which screwed all my nerves.”
In February, Osbourne gave an update on his health, sharing that he could no longer walk.
“I may be moaning about how I can’t walk as well,” he said in an interview with SiriusXM, “But as I look down the road, there’s people that didn’t do half as much as me, and they didn’t make it.”
Thousands gathered in the streets of Birmingham last Wednesday ahead of Ozzy’s funeral procession, led by his wife, Sharon Osbourne, and their kids Jack and Kelly.

Fans crowded behind barriers through the “Crazy Train” rocker’s hometown, while tens of thousands watched a recorded livestream of the procession , which featured a marching band — donning Ozzy Osbourne jackets — played covers of his hits.
Kelly and Jack escorted Sharon as she tearfully laid flowers atop hundreds of other bouquets at what appeared to be Black Sabbath Bridge. Next to the tributes was a sign that reads, “Ozzy Forever. Birmingham will love you forever.”
Along with Kelly, Jack and Shthe aron, to whom he was married since 1982, Ozzy is survived by their oldest child, Aimee, as well as the three children he shared with his first wife, Thelma Riley.
